How this tracker works (and the line we draw)

Where the grades come from. The 15 questions and their weights are derived from publicly available state medical board disciplinary orders, state pharmacy board enforcement actions, DEA bulletins, FDA guidance, and current statutes. We track these continuously through our regulatory intelligence system and consolidate them into one place so operators don't have to read 50 state board newsletters to know where the risk is.

The role AI plays. AI helps us summarize and condense the source material into usable assessment questions. The scoring itself is deterministic — same inputs always produce the same score, and the rubric is published below in the questions you'll see. There is no AI black box deciding your grade.

What this is not. TrueEval is not a law firm or an accounting firm. This tool is informational; it is not legal, accounting, or medical advice, and it does not create an attorney-client, accountant-client, or provider-patient relationship. If your results surface material gaps, consult qualified healthcare counsel licensed in your state before making business decisions.

Why hormone optimization compliance is uniquely hard

DEA + state CDS + PDMP — three layers

Testosterone is Schedule III. Prescribing requires DEA registration, state CDS in most states, and PDMP reporting. Missing any one layer is a frequent enforcement trigger.

FDA peptide reclassification

BPC-157, TB-500, and several other popular peptides were moved to FDA Category 2 (no permitted compounding) in 2023-2024. Operators continuing to compound them face federal prosecution. Most don't know about the change.

Ryan Haight Act + DEA flexibilities

The DEA's pandemic-era telehealth flexibilities have been extended quarterly but may revert. Programs without a contingency plan face operational disruption when rules change.

"Research grade" = federal crime

Research-grade peptides are not approved for human use. Prescribing or dispensing them constitutes the distribution of unapproved drugs — a federal offense the FDA has prosecuted in 2025-2026.

FAQs

Is this free?

Yes. The tracker is free as a public resource. We want TRT, peptide, and hormone clinic operators to know whether they're exposed to DEA, state board, or FDA action before they get a knock on the door. Your results are private to you.

Why is hormone optimization compliance higher-risk than other telehealth?

Testosterone is Schedule III (DEA registration required + state CDS in many states + PDMP reporting). The DEA Ryan Haight Act requires an in-person exam for controlled substances unless pandemic-era flexibilities apply. FDA reclassified several popular peptides (BPC-157, TB-500) to Category 2 in 2023-2024 — meaning compounding them is now a federal violation. Hormone clinics live at the intersection of federal + state enforcement.

What about peptides — are they legal?

Some peptides remain on the FDA's Category 1 list and can be legally compounded with patient-specific prescriptions through a 503A or 503B pharmacy. Others (BPC-157, TB-500, IGF-1 variants) are on Category 2 — compounding for human use is a federal offense. The tracker flags whether your program is operating within current FDA permitted-compounding rules.

Is my data shared with DEA or state PDMP?

No. Your responses are private. We do not share individual assessment data with the DEA, state PDMPs, FDA, or any regulatory body. Email is optional, only required if you want the downloadable PDF.

What if I get an F?

Hormone clinic enforcement is among the highest in 2025-2026. Programs without state CDS registrations, phantom medical directors, research-grade peptides, or async-only Schedule III prescribing are the explicit targets. Every gap is fixable, but timing matters — take the full Compliance Assessment this week if your grade is C or below.
